Internship/Student Teaching Semester - Spring 2009
Complete the following prior to Internship/Student Teaching:
- Internship Agreement (PDF) - signed copy due in Killian 228 prior to beginning Internship. (Undergraduates not employed in school)
- Health Certificate (PDF) - take completed form to school if not already employed by the school system.
- Background Check (PDF) - within six months prior to beginning Internship. Needs to be on file with the Office of Field Experiences unless student is already employed by the school system.
- Site Supervisor Designation Form (PDF) - Only if employed at school where completing internship.

Fall 2009 - Internship/Student Teaching Watch your catamount e-mail for forms and dates. The application deadline will be Friday, February 27.
Undergraduate K-12 licensure students who will begin Internship or Student Teaching Fall 2009 have a mandatory orientation. Attend one of the following sessions:
- Monday, January 26th at 5:00pm
- Tuesday, January 27th at 8:30am
- Wednesday, January 28th at 8:30am
- Thursday, January 29th at 5:00pm
Graduate, licensure only and BK program students should meet with their advisor in lieu of the orientation session
Licensure Requirements during Student Teaching/Internship Experience
All Students:
- Grade of C or better in all associated internship and seminar courses.
Initial Licensure (Except BK):
- Completion of Individual Growth Plan (IGP) and submission on TaskStream with a score of 3 or 4.
- Completion of Teacher Work Sample and submission on TaskStream with a score of 3 or 4.
- Completion of Basic Competencies Exit Criteria with a score of 3 or 4. This form MUST be signed by student, cooperating teacher, and university supervisor and also by the academic supervisor, if applicable. Exit Criteria Scores also be entered onto TaskStream by the university supervisor. Lateral Entry teachers should have this signed by their mentor as the cooperating teacher.
Advanced Licensure (Except BK):
- Completion of TaskStream portfolio as directed by university supervisor.
- Completion of Advanced Competencies Exit Criteria with a score of 3 or 4. This MUST be signed by student, cooperating teacher, and university supervisor. This must also be entered onto TaskStream by the university supervisor.
BK Students:
- Completion of TaskStream portfolio in conjunction with your university supervisor.
